What is involved?

Please contact us by telephone or email if you would like to get involved. The information we would require would be the hospital at which the participant is seen unless you are a mother (in which case the hospital at which your child is seen), your age, your address and how you think you can help (a blood sample via your GP or a saliva sample). We can then make contact with you by post.
We will send you a copy of the information leaflet to keep, a consent form and a brief questionnaire and a freepost reply envelope. Sample print copies of these forms can be found here; alternatively you can view an HTML version here.
We will need precise information about your or your child’s type of heart problem. The easiest way to give us this information is to send us a copy of a letter from the hospital describing the heart condition. Alternatively we could ask you the necessary questions by telephone or we could contact your heart doctor to find out this information.
